PA NEN Nutrition Education Database

Thank you for taking the time to complete this survey. The information you provide will be added to a web-based, state-wide database listing organizations that are currently involved in nutrition education. This database will provide at-a-glance information that can be used to find educational programs in specific locations. It will also be utilized to locate gaps in services and to reveal agencies that could be linked together to more efficiently provide service to those in need.

To add your organization to the database, please enter your Contact Information on the first page of the survey. Next complete the Nutrition Education Program Information for each program provided. If your organization offers multiple programs, please complete the survey for each individual program.

*To qualify for inclusion in this database, the program must adhere to the 2005 Dietary Guidelines for Americansand MyPyramid. Additionally, at least one of the components listed on page 2 of this survey must be offered at no cost to participants.

Upon approval, your program information will be added to the database and posted online.

In an effort to ensure that the database remains current, all listed organizations will be required to update their information on an annual basis. An e-mail will be sent to your contact requesting that the survey be completed by an allocated timeframe. In the event that the information is not updated in this timeframe, the program will be removed from the database.

The database will be managed by the Pennsylvania Nutrition Education Network (PA NEN). The PA NEN facilitates communication among those individuals, organizations, and agencies concerned with community nutrition education. The Network mission is to ensure that effective and appropriate nutrition education is available, primarily for low-income populations, across the state.
